Scorers: Reijnders 16'; Dybala 23'
This was as tense an encounter as it could have been between two Serie A giants. Milan led the way in shot attempts (21 to 12), but Roma controlled the ball in this end-to-end affair.
The opener arrived at the hands of the hosts when Tijjani Reijnders scored his ninth of the season after racing from midfield and slamming a ball home at the back post.
The lead, however, didn't last long for Milan, and Roma struck back with a beautiful goal less than 10 minutes later.
Artem Dovbyk pulled off a brilliant backheel flick, and the ball fell perfectly for Paulo Dybala, who volleyed home beyond Mike Maignan.
Milan manager Paulo Fonseca, meanwhile, was also shown a red card for dissent just before half-time after arguing over a non-penalty call on Reijnders.
Things remained deadlocked as players fell short of genuine chances in the second half.
Two impressive saves from goalkeeper Mile Svilar on Ismaël Bennacer and Samuel Chukwueze notably frustrated the hosts.
The final 15 minutes of action were cagey, and Roma fell inches short of stealing all three points after hitting the crossbar in the 89th minute.
